Applications can be picked up and completed at our office. It should take no more than 15 minutes if you come prepared. Applications will not be available after 3:00pm. 
Please bring the following documents with you:
-Your valid Driver's license or ID
-Your Social Security Card or Birth Certificate
-Your High School Diploma, School Transcripts, or GED 
We are open:
Monday through Friday from 8:00am-4:00pm at the SMART JATC Training Center.
